The medical form has to be filled out annualy or if anything changes with the youth.

YOUTH STANDARDS-GENERIC
This is the Generic Youth Standards form.  This form will need to be signed and turned in along with the Medical Release Form.  This form is the standards form for any local event.  If there are special trips, we will have a seperate stadards form for you to fill out.
